The field record
Templates for the record the whole report rests on: walk-through survey notes, the limitations and field conditions statement, and the document review and interviews log — each with a worked example.
Everything in a PCR is downstream of a day on site. What was seen, what was closed, what the maintenance manager said, what was requested and never provided — that record is what makes the report defensible, and it is the part most often compressed into a sentence.
The limitations statement in particular is treated as boilerplate and is anything but. A limitation that names what was unobservable and why is a professional judgement; the same limitation left generic reads as an excuse written in advance.
